Authors, alternate authorship types

Requirements:
- If an author is a collaborative group, such as an institution or working group, then contain the collaborator within <collab>
- For collaborative authorship, the group member names will be included. List these within <p> in an author footnote (shown below; see also author footnotes)
- In cases of collective authorship, tag the name representing the collective group within <collab> with the attribute 'collab-type' set to "collective"
