For CJC only: If a reference or reference part contains a footnote, then set @publication-type="ref-fn" (See example 2b)
For CJP only: the order of elements within <mixed-citation> for a journal reference in the Candian Journal of Physics should be <comment>, <person-group>, <source>, <volume>, <fpage>, <year>, <pub-id>
